Organizations can duplicate an existing submission form to use it as a starting point for a new form. 

  1. Sign in to your organization's account and click on Forms in the upper-right corner of the screen.
  2.  On the Forms page, find the form you wish to duplicate and click the Duplicate button to the right of the form's name.
  3. Enter the name of the new form. Then, click Duplicate Form.
  4. All settings and form fields from the original form will be copied over to the new form. Click on the new form to make edits to it as needed.
